【摘要】 目的 探讨术中皮层躯体感觉诱发电位(CSEP)监测客观评价脊髓功能及完整性并判断预后的临床价值。方法 对54例脊柱脊髓手术患者,实施手术前、中、后CSEP监测,观察并分析皮层电位的潜伏期及波幅,波幅降低≥50%或潜伏期延长≥10%为明显改变即报警标准。结果 真阴性72.22%(39/54),术中潜伏期及波幅均无异常变化,术后无新的神经功能障碍。假阴性0.18%(1/54),术中CSEP无明显变化,但术后患者出现新的神经功能障碍,真阳性25.93%(14/54),手术过程中CSEP出现短暂潜伏期延长小于10%、波幅下降小于50%,暂停操作或改变操作方向后波形恢复或改善。结论 CSEP的监测在脊柱手术中对脊髓的牵拉、缺血以及损伤很敏感,术中及时干预,可使CSEP改善;CSEP能较好地反映脊髓功能状态及完整性,对防止脊髓损伤、判定神经功能预后有重要价值。

5Vv_x2jF0XT]/@AZ:zn0 Abstract:Objective To explore the clinical value of somatosensory cortex evoked potential(CSEP)monitoring in objective evaluation for spinal cord function and integrity,as well as judgement for the prognosis of surgery.Methods CSEP monitoring was performed before the operation,during surgical procedure and after the operation in 54 cases of spinal cord surgery patients.The latency and amplitude of cortical potential were observed and analyzed. Amplitude reduces ≥ 50% or prolonged latency≥10% were taken as scilicet alarming standards.Results True negative finding was 72.22%(39/54)where both the latency and the amplitude without abnormality changes during the operation and without new neural functional deficit after operation.False negative finding was 0.18%(1/54)where the latency and the amplitude without abnormality changes during surgical procedure and with new neural functional deficit after operation.True positive findings was 25.93 %(14/54)where transient prolonged latency<10% and reduced amplitude<50% during surgical procedure were observed. After suspending procedure or changing direction during surgical procedure,the waveform restored or improved.Conclusion CSEP monitoring during surgical procedure have sensitiveness of distracting spinal cord,ischemia and injury.In a timely manner of interference will improve the monitoring of CSEP.CSEP have an important value in preventing spinal cord injury and evaluating the prognostic of neural function because CSEP can be better reflect the function and integrity of the spinal cord.
